Vascular Plug-Assisted Retrograde Obliteration (PARTO) of Right Mesorenal Shunt for Hepatic Encephalopathy: An Uncommon Site of a Spontaneous Portosystemic Shunt in Cirrhosis

چکیده
A 47-year-old male patient with alcohol-related cirrhosis presented to the liver clinic complaint of repeated episodes hepatic encephalopathy (HE) manifested as a disturbance in sleep pattern, hypersomnia, impaired memory, mental confusion, and drowsiness (West Haven grade II/III). On evaluation, had Child-Pugh score 6 (CTP-A), hyperammonemia (>200 µg/dL), small low-risk esophageal varices, splenomegaly, large right mesorenal shunt (MRS) afferent from superior mesenteric vein draining into renal ([Fig. 1]). Vascular plug-assisted retrograde transvenous obliteration (PARTO) MRS was planned since HE persisted despite optimization standard medical treatment.

Plug-Assisted Retrograde Transvenous Obliteration of Spontaneous Splenorenal Shunt for Refractory Hepatic Encephalopathy: Case Series
Intervention treatment such as balloon retrograde or anterograde transvenous obliteration has been used for management of refractory hepatic encephalopathy as well as gastric variceal bleeding. Recently, plug-assisted retrograde transvenous obliteration without a help of balloon was newly developed to treat these patients. Here, we report three cases suffering refractory hepatic encephalopathy ...

While balloon-assisted retrograde transvenous obliteration (BRTO) has been used for two decades in Asia for the management of gastric variceal bleeding, it is still an emerging therapy elsewhere. Given the shunt closure brought about by the procedure, BRTO has also been used for the management of portosystemic encephalopathy with promising results. Modified versions of BRTO have been developed,...
متن کاملPathological Predictors of Shunt Stenosis and Hepatic Encephalopathy after Transjugular Intrahepatic Portosystemic Shunt
Background. Transjugular intrahepatic portosystemic shunt (TIPS) is an artificial channel from the portal vein to the hepatic vein or vena cava for controlling portal vein hypertension. The major drawbacks of TIPS are shunt stenosis and hepatic encephalopathy (HE); previous studies showed that post-TIPS shunt stenosis and HE might be correlated with the pathological features of the liver tissue...
متن کاملSevere Hepatic Encephalopathy after Transjugular Intrahepatic Portosystemic Shunt (TIPS): Value of Shunt Reduction and Occlusion
Hepatic encephalopathy (HE) after implantation of the transjugular intrahepatic portosystemic Shunt (TIPS) is generally well treatable. Severe and refractory HE might be treated with shunt reduction or occlusion. We performed a retrospective study between January 2004 and January 2016. Out of 456 TIPS implantations, 17 patients (3,7%) were treated with shunt reduction and 6/17 patients with add...
متن کاملCongenital intrahepatic portosystemic shunt with variant inferior right hepatic vein.
We present a case of congenital intrahepatic aneurysmal portosystemic shunt in which the right portal vein communicates with both a variant inferior right hepatic vein and the right hepatic vein. We also describe the importance and efficacy of three dimensional computed tomography reconstruction images in determining subtle, small shunt vessels.
